- Connect Input-5 to VCC Port.
- Connect GND Port to Input-12.
- Connect Output-4 to Input-1.
- Connect Output-3 to Input-3.
- Connect Output-2 to Input-8.
- Connect Output-1 to Input-10.
- Connect Output-5 to Input-11.
